  Mrs. Desai 45 yrs was referred to us for opinion by a renown doctor of Surat. She has been a known case of type 2 diabetes mellitus and hypertension since 12 years. Her BMI was 37 and had morbid obesity. So thus she was a patient of Metabolic Syndrome. The issue with her was that her sugars were uncontrolled and now not even responding to medication and Insulin Injections. Also she had an umbilical hernia. Patient was advised Laparoscopic sleeve Gastrectomy with laparoscopic cholecystecomy with Umbilical hernia repair. Patient came for follow-up on 7th day and is off  her Insulin injection with only 1 medication each for Diabetes and Hypertension which hopefully will also stop as time progress with the weightloss. Laparoscopic Sleeve gastrectomy was offerred  to this patient for management of her obesity ( BMI 46). Patient was from Dhulia (Maharastra) and had specifically visited Surat for treatment. She is hypertensive and has hypothyroidism. She had history of previous 4 abdominal surgeries which was a challenge to us considering the adhesions open surgery creates. Patient also had large gall stone (22mm) and wanted removal in the same sitting. Attached are the specimen of remnant stomach and the gall bladder. She was discharged on the 2nd day after surgery.
